People with chronic physical pain are invited to submit up to five art images of pain-related art.  Only the highest quality images will be used.

In addition to the art, please include title of each piece, type of media and dimensions (LxWxD). Also, you must submit a brief written description of how each piece of art relates to you and your chronic pain.  Please refer to art images and statements on this website for examples. Finally, include complete contact information - full name, street mailing address, email address, and phone number.
